Designed to support RTM workflows—from documentation to billing

Remote Therapeutic Monitoring (RTM) is a set of CPT codes for the work you already do between appointments — reviewing what your client reported, watching for patterns, and adjusting the plan. Medicare recognizes these codes; commercial coverage varies by payer and plan.

Your client logs meals in their app. You review those logs in yours. Well Plated tracks that review time automatically, totals it by client and calendar month, and keeps a record of the conditions you’ve confirmed — so the documentation is ready when your billing team needs it.

Payers set conditions on RTM, and meeting them is up to you and your billing team:

  • Monitoring data. Enough days of client-reported data in the month.
  • Review time. At least 20 minutes of your review and management time, with a shorter tier for 10–19 minutes.
  • A live conversation. At least one real-time, two-way conversation with the client or caregiver during the month — a phone or video check-in. In-app messages don’t count.
  • Enrollment and consent. Plus any payer-specific requirements.

When those conditions are met, you may be able to offset some or all of your subscription cost.

Is the app for people in recovery free?
Yes. The app for people in recovery is free during Early Access and will remain free afterward.
When do subscriptions begin?
Everything is free during Early Access. We have not set a billing date, and we will notify members before any paid subscription begins.
Will the prices change?
Planned pricing is shown for reference and may change before subscriptions begin. Early Access members will receive preferred introductory pricing.
How is provider pricing calculated?
The Professional plan includes up to five active clients. Additional active clients are planned at $8 per month each, so providers pay only for clients they are actively supporting.
Do caregivers pay more when a provider joins?
No. A caregiver’s price does not increase when a provider is connected to the same loved one. Providers use their own provider membership.
What is RTM, and how does Well Plated support it?

What does a month need to include to support an RTM claim?

Does Well Plated submit RTM claims?
No. Well Plated tracks and documents provider review time by client and month, and provides a summary for the billing team. Submitting claims stays in the provider’s existing workflow.
Is RTM billed to the client’s insurance?
Yes. RTM is billed like other covered services, so normal cost-sharing such as copays or deductibles can apply. It is worth raising with clients before enrolling them.
